Quarterly Performance Report (Excel) – due to the City on a quarterly basis
Grant Close Out Report (Word)
Note that additional reporting will be required to accompany requests for payment on a monthly basis and substantiate expenses for City reimbursement. Some of this reporting may include other beneficiary information (e.g. if rental assistance or flex funding is used).

Self-Evaluation Form (PDF) – due every 3 years
Accessibility Checklist (PDF) – due every 3 years
Yearly Progress Report (PDF) – due each year a Self-Evaluation Form is not submitted
